ESSENTIAL JOB DUTIES:
- Personal care duties include but are not limited to bathing, grooming, feeding, scheduling appointments, turning and positioning.
- Assist the patient with transfer, ambulating, prescribed exercise as instructed by the Home Care Nurse.
- Observe and report specific vital signs and symptoms as instructed by Home Care.
- When instructed by the Home Care Nurse, record weight, check client's pulse, temperature, respiration rate, measure food and fluid intake and output.
- Prepare modified diet as ordered by the home care nurse.
- Record keeping such as maintaining records on client, services received, current conditions and progress.
- Maintaining a safe, secure and healthy patient environment, by following the asepsis standards and procedures.
- Maintaining client and family confidentiality.
QUALIFICATIONS:
- Have completed a training program approved by the New York State Department of Health as a Home Health Aide or be enrolled as a trainee aide in New York State approved training program under the supervision of qualified RN.
- Be a minimum of 18 years of age.
- The ability to read and write in English.
- Have a physical examination satisfying the New York State Department of Health requirements for Home Health Care Workers.
- Submit annual mandatory compliance documents such as annual physical, QuantiFERON or PPD, a copy of x-ray report (if positive PPD) and annual TB questionnaire.
- Complete 12 hours of in-service annually.

LICENSE/CERTIFICATION:
- PCA or HHA Certification.
