Home Healthcare: We provide physician-ordered skilled nursing and health care services to homebound patients recovering from illness or injury. Position Summary:
Responsibilities of the home health aide include, but are not limited to, the following:
- Providing personal care including:
- Baths
- Back rubs
- Oral hygiene
- Shampoos
- Changing bed linen
- Assisting patients with dressing and undressing
- Skin care to prevent breakdown
- Assisting the patient with toileting activities
- Keeping patient's living area clean and orderly, as appropriate
- Planning and preparing nutritious meals.
- Assisting in feeding the patient, if necessary.
- Taking and recording oral, rectal and axillary temperatures, pulse, respiration and blood pressure when ordered (with appropriate completed/demonstrated skills competency).
- Physical Requirements:
- Ability to work in fast paced environment with multiple interruptions.
- While performing the duties of this Job, the staff is regularly required to stand; walk; use hands to finger, handle, or feel; reach with hands and arms and talk or hear. The staff is occasionally required to sit; climb or balance and stoop, kneel or crouch.
- The staff must regularly lift and/or move up to 25 pounds, frequently lift and/or move, working in teams and assistive devices, up to 50 pounds and occasionally lift and/or move, working in teams and assistive devices, up to 100 pounds.
- Specific vision abilities required by this job include close vision, color vision, depth perception and ability to adjust focus
- Assisting in ambulation and exercise according to the plan of care.
- Performing range of motion and other simple procedures as an extensional therapy service as ordered (with appropriate completed/demonstrated skills competency).
- Assisting patient in the self-administration of medication.
- Doing patient's laundry, as appropriate.
- Meeting safety needs of patients and using equipment safely and properly (foot stools, side rails, etc.).
- Reporting on patient's condition and significant changes to the assigned nurse.
- Adhering to the Organization's documentation and care procedures and standards of personal and professional conduct.
Minimum Qualifications:
- Current CHHA license
- Minimum of 1-year nursing experience required
- 1 year of Home Healthcare / Hospice experience is preferred
- Current CPR card for Healthcare Providers
- Strong organizational and communication skills
- Ability to travel per job requirement'
- Must be a licensed driver with an automobile that is insured in accordance with state and organization requirements and is in good working order.
