PositionOverview : The Home Delivered Meals Driver - Lead transports prepared meals to home-boundeldersandcommunity members with a disability within a group ofgeographicareas.The driver performs a wellnesscheck for eachconsumerandserves as a link betweenAgeSpanandtheconsumer.
OrganizationalRelationships
- Reports to the SiteSupervisor and Nutrition Program Manager.
- Collaborateswith allNutrition Departmentstaffmembers.
EssentialJobFunctions
- Reports to location needed that day/supervisor of that location location.
- Ensures that meals are safelypackaged in approvedfoodcontainers to maintainproper holding temperatures.
- Reports any issuesregarding poor or unsafefood quality.
- Loads mealcontainersintopersonal vehicle.
- Follows all traffic andsafetylawswhile on duty.
- Delivers meals to consumersaccordingto a predetermined route.
- Delivers and sets up meals for consumersintheirhomewhen necessary.
- Ensures thatconsumerswithspecificdietaryrequirementsreceivethecorresponding meal.
- Conforms to any specific delivery instructionsnoted for the consumer.
- Upon meal delivery,the driver confirmsthatthe consumer is athomeandreports any unusual circumstances.
- Performs a wellnesschecks by deliveringeachmealdirectly to the consumer.
- Recordseach meal delivery on theroute sheet.
- Reports any issues or concernsregarding the consumerimmediately to the officeand records.
- Recordsandreports if a consumer is not at homewhenthe meal delivery is attemptedand discards the Meals may only be consumed by theintendedrecipient.
- Communicatesroutineinformationreceivedfromtheconsumerandfamilymemberstoappropriateagency individuals
- Demonstratesflexibilitywithchanges to delivery schedules or routes.
- Other related duties as assigned.
Non-essentialJobFunctions
- Attends meetings as required, including staffmeetings,in-servicetrainings,utilizationreviews, and collateralagency meetings.
Requirements
- Able to read, write,understand,andinterpretbasic
- Must have owntransportation, current driver's license and must show proof of automobileinsurancewith a minimumliabilitycoverage of $20,000/$40,000.
- Requires lifting and carryingup to 50 pounds, getting in and out of a vehicle up to 30 times per day, bending, reaching, twisting, walking, and climbing multiple flights of stairs at residences that do not have elevators.
- Requires driving in four season weather conditions.
