Job Summary:
Under the direct supervision of the Center Manager, serves as Home Care Team Leader responsible for the implementation of home care services for program participants, including completing home safety reviews, coordinating basic durable medical equipment, and assessing and scheduling in-home personal care support. Serves on Interdisciplinary Team and oversees home care team.
Duties/Responsibilities:
- Complete the Home Care Assessment at initial enrollment, semi-annual reassessments and for unscheduled reassessments as needed.
- Assess home care needs in response to service requests.
- Performs supervisory visits in accordance with state and federal regulations and if surveillance is required more often.
- Complete all documentation in electronic medical record.
- Presents recommendations for home care to the Interdisciplinary Team
- Develop individualized care plan in collaboration with the interdisciplinary care team.
- Collaborate with other PACE team members, including nurses, social workers, and therapists, to coordinate care and address participant needs in the home setting.
- Evaluate personal care service needs and documents in home care service plan.
- Ensure home care schedules are staffed appropriately per the participant's Care Plan.
- Coordinate and monitor contracted home health aide services.
- Participate in Quality Improvement Initiatives when directed.
- Order/track basic Durable Medical Equipment in EMR per policy and procedures
- Attends the IDT meeting to represent participant care needs of in the homes.
- Other duties as assigned.
Supervisory Responsibilities:
- Lead and oversee the home care team including home health aides and home care scheduler, providing guidance, support, and regular performance feedback.
- Conduct ongoing training and professional development to ensure that home health aides are equipped with the necessary skills to provide safe and effective care.
- Verify Home Care timesheets, mileage logs and submit to payroll in a timely manner.
Required Skills/Abilities:
- Ability to travel to and perform home visits throughout the New Orleans and the metropolitan area.
- Relates well with people of all ages but has a deep sensitivity and commitment to the elderly.
- Ability to organize work and follow through on assigned tasks.
- Excellent verbal and written communication skills.
- Must possess the ability to make independent decisions when circumstances warrant such action.
- Must be able to deal tactfully and patiently with personnel, participants, family members, visitors, government agencies/personnel, and the public.
- Must have the cognitive ability to process and perform tasks for participant safety.
- CPR and First Aid Certification.
- Must be a minimum of 18 years of age.
Education and Experience:
- Bachelor's degree in nursing or healthcare related field. Will consider LPN, COTA, MA, or other discipline with related experience in lieu of education requirements.
- One year of experience working with the frail and elderly population and at least one year of prior supervisory experience.
- Must possess a valid LA driver's license, vehicle, and clean driving record.
- Good interpersonal skills and the ability to work with geriatric patients and handle unusual interpersonal situations.
Physical Requirements:
- Must be able to move intermittently throughout the workday.
- Must be able to lift, push, pull, and move a minimum of 25 pounds.
All candidates for employment with CCANO must submit to a criminal records check as a condition of employment.
New employees will be required to complete a criminal background check prior to employment.
Compliance with the State of LA Child Protection Act and Archdiocese Policy on Abuse and Neglect, to provide for the safety and security of all clients served by CCANO programs.
