The Home Care RN Case Manager is responsible for coordinating and managing patient care in a home setting. Key responsibilities include:

Conducting comprehensive patient assessments

Collaborating with interdisciplinary teams and family members

Providing education and support to patients and caregivers

Qualifications:

Current RN license in Massachusetts

Minimum of 2 years nursing experience, preferably in home care
