Several Immediate PRN positions available to RNs with Home Care Experience.
Our client is a Medicare certified and Pennsylvania state-licensed home health care providing individualized skilled care in the comfort of your home is looking for an Home Care Registered Nurse
Type: Contract
Schedule: Day shift, Monday to Friday, Weekends as needed
Location: Philadelphia, PA
Duties:
• Follow patient's plan of care in accordance to doctor's orders
• Travel to patient homes as assigned and manage own schedule
• Home care visit duties may include: medication administration, wound care, catheter changes, disease management and education
• Monitor, observe, evaluate and manage patients' conditions, report to physician and properly document
• Perform patient assessments, evaluations and Start of Care Admissions
• Case manage, follow up and coordinate with care team, family and physician
Qualifications:
• A current license as a Registered Nurse in good standing and able to practice in Pennsylvania.
• A minimum of one year of recent, verifiable RN clinical (medical/surgical) experience.
• A Minimum one year prior home health care experience providing home visits
• Graduation from an accredited and approved nursing program
• Strong interpersonal skills, excellent work ethic and communication skills
• Experience with electronic medical records (EMR) preferred. Knowledge of Axxess software is a plus.
• Knowledge of OASIS documentation
• Knowledge of Medicare requirements and compliance
• Current CPR certification
• Clear criminal background history and clear drug screen
• Associate (Required)
• 2 years of Home Health Care Experience required
• 1 year of Acute care experience preferred
• 1 year of Nursing experience required
