Home Care Assistant STNA - University Hospitals

• Provides patient care, under the supervision of a Case Manager: Nurse or therapist.
• Provides personal care/companionship as directed by plan of care
• Provides light housekeeping/ light meal prep tasks as directed by plan of care
• Effectively communicates both verbally and in writing
• Initiates and maintains positive relationships with patients/customers and co-workers
• Observes and reports patient status changes including skin and psychosocial
• Takes responsibility for self-development and supports a learning environment
• Displays commitment to the mission of UHHS/UHHCS and its values
• Performs other duties as assigned.
• Complies with all policies and standards.
• For specific duties and responsibilities, refer to documentation provided by the department during orientation.
• Must abide by all requirements to safely and securely maintain Protected Health Information (PHI) for our patients.
• Annual training, the UH Code of Conduct and UH policies and procedures are in place to address appropriate use of PHI in the workplace.
• The position will cover 2 days in Ashtabula and 3 days in Portage (the cities are Kent, Mogadore,
• Brimfield, Rootstown, Ravenna, Atwater, Deerfield, Newton Falls, Windham, Garrettsville, Hiram, Lake Milton, Streetsboro) counties. 
• The position is in Home Care Services.
• 100% travel to visit patients at their homes.
• Monday-Friday. 
• This position is part time 24 Hrs. per week. 

• High School Equivalent / GED Required.
• Completes required continuing education requirements to maintain certification.
• 1+ years Home care experience preferred
• State Tested Nursing Assistant (STNA) or equivalent certification required.
• Basic Life Support (BLS) within 30 Days.
• OH Driver's License (Valid) with car insurance coverage required.
• Successful completion of agency written test and competency evaluation of skills in the field within 30 Days.
