Full-Time Home Care Aide, STNA for Hospice
Are you a dedicated and compassionate Home Care Aide, STNA looking to make a real difference in patients' lives? The Visiting Nurse Association of Ohio is seeking an experienced and motivated Home Care Aide, STNA to join our dynamic Hospice team serving the Cuyahoga County area. This is a full-time position offering the opportunity to work in a supportive, community-focused environment that values your expertise. The Home Care Aide, STNA is directly responsible to the Home Care Aide Supervisor for day-to-day scheduling, to the Case Manager for specific plans of patient care and direct field supervision and to Clinical Manager for evaluation.
Responsibilities:Essential Functions:
The Home Care Aide’s primary duties include but are not limited to:
Personal Care Assistance:
- Provide assistance with activities of daily living (ADLs), including bathing, grooming, and dressing as outlined in the Home Care Aide Plan of Care.
Meal Preparation:
- Plan, prepare, and assist with nutritious meals that meet the dietary requirements specified by the patient’s care plan.
Environmental Safety:
- Ensure a safe, clean, and healthful living environment for the patient. Perform light housekeeping and other tasks that help maintain the patient’s well-being.
Treatment Assistance:
- Assist with prescribed treatments and therapies, under the guidance of a Registered Nurse, within the scope of the aide’s education and training.
Patient Monitoring:
- Observe and document any changes in the patient's physical or mental condition, immediately reporting significant changes to the Registered Nurse.
Record Keeping:
- Maintain accurate and timely records of care provided, including activities performed, observations made, and interactions with the patient.
Flexibility & Teamwork:
- Accept new patient assignments and assist in substituting for absent Home Care Aides as needed. Participate in patient care conferences and in-service training sessions.
Customer Service:
- Ensure the highest level of patient satisfaction by promoting a positive and caring environment. Provide emotional support through friendly, stimulating social interaction.
Professional Development:
- Participate in in-service programs, training sessions, and patient care conferences to enhance professional knowledge and skills.
Qualifications and Requirements:
- Education: High School Diploma or GED
- Experience: Minimum of six months in a supervised healthcare setting, Hospice experience preferre
- Certifications:
- STNA Certification (existing VNAC non-certified staff will be grandfathered)
- CPR Certification (preferred)
- Must complete 12 hours of training annually
- Licenses: Valid Driver’s License

- Strong interpersonal skills, capable of working with individuals from varying cultural, social, and ethnic backgrounds
- Ability to work independently and responsibly with minimal supervision
- Strong organizational and time management skills
- Ability to maintain strict confidentiality at all times
