The Sisters of St Joseph, located at the Provincial House in Latham, are seeking staff to join our team in providing for Sisters who are independent but who require some support and care. The Holistic Care Partner is responsible for coordinating health and wellness services for members of the Albany Province. The Holistic Care Partner works in collaboration with other members of the health and wholeness team, and community life team, with the goal of enhancing the ability of sisters living in diverse settings to maintain health of body, mind and spirit.
- Oversees and facilitates care plans for sisters living independently with frequency of in person visits determined by level of independence / acuity.
- Works in partnership with sisters to proactively anticipate needs such as changes in living situation, medical equipment to support independent living, other services / supports for daily living activities: housekeeping, meal preparation, transportation.
- Assists sisters during times of acute illness, performing clinical assessments to define appropriate time course and level of care needed
- Assists sisters who are members of the Albany Province during times of care transition (home, hospital, rehabilitation facility). This may require traveling to visit sisters in hospitals, nursing homes and other care facilities throughout the province
- Acts as the healthcare liaison with the PACE program in Albany / Onondaga Counties (others as added)
- Partners with sisters to develop their own care plans to prevent disease exacerbation, improve outcomes, increase their engagement in self-care, decrease risks, and improve physical, mental, emotional and spiritual health and works in partnership to implement a holistic plan of care
- Assists sisters in navigating the healthcare system, coordinating their specialty care, following-up on test results and other care coordination needs.
- Develops a cohesive and strong team-oriented relationship with other team members (Community life team, social worker, spiritual advisor, benefits) and works in partnership to facilitate a holistic plan of care
- Documents the plan of care for access by members of the interdisciplinary team.
Qualified candidates will have the following skills:
- Ability to travel to and from locations throughout New York State as needed (must have a valid, current driver’s license)
- Valid LPN or RN License
- Able to demonstrate strong clinical nursing assessment skills
- Minimum of 3 years of related experience in an environment where clinical nursing skills, care coordination and care management skills were required
- Affinity for innovation, creativity and creating good change
- Ability to work a flexible schedule including weekends and holidays
- Good Communication skills both verbal and written
- Experience in working with geriatric patients or persons and the process of aging
- Some understanding of the culture of woman’s’ religious communities preferred
- Able to demonstrate strong teamwork and support a positive work environment
