The High Needs Case Manager II acts as a peer mentor for newly hired staff, which includes allowing new staff to shadow their appointments, and will be available for questions if management is not available. This position also provides High Needs Case Management services to assigned clients, with a caseload that follows AHCCCS guidelines. The HNCM II will follow timelines for weekly contact, home/community visits, attending medication checks, and other required duties.
Responsibilities:- Assists clients assigned in navigating discharge from hospitals, assists in finding intensive services as clinically indicated, and working to decrease behaviors so client can be stable in the community and home.
- Completes referrals, prior authorizations, and other paperwork required by the health plans to get client the services that are clinically indicated on their Integrated Service Plan.
- Responsible for coordinating and facilitating Child and Family Team (CFT) Meetings.
- Collaborates with clinical team to ensure that Integrated Service Plans, Child and Family Team (CFT) Meetings, CASII/ECSII, DUG, and other required paperwork is completed accurately and current.
- Collaborates and coordinates services with other involved agencies, such as Department of Child Safety, Probation and Parole Departments, Division of Developmental Disabilities, schools and other providers.
- Works effectively within the Arizona Recovery Model as outlined by the 12 principles including: Accessible services, Best practices, Collaboration with families, Collaboration with others, Connection to natural supports, Functional outcomes, Independence, Most appropriate setting, Respect for the child and/or family's unique cultural heritage, Services tailored to the family, Stability, and Timeliness.
- Required to get trained and credentialed to complete Comprehensive Assessments, and complete assessments as assigned.
- High School Diploma/GED required.
- Bachelor’s Degree preferred.
- Notable Behavioral Health experience, with direct HNCM experience highly preferred.
- Supervisory experience preferred.
- Must be able to perform the essential functions of the job including prolonged sitting and in certain positions driving, lifting 20lbs, walking, standing and executing dexterity with a keyboard or smart pad.
