About the Job
Niagara Hospice is looking for a compassionate, certified Home Health Aide (HHA) to join our Home Care team. The HHA will provide in home HHA services to patients throughout Niagara County under Niagara Hospice care.
Schedule: Full Time Monday - Friday 9am-5pm, with every other weekend required
Location: On the road within Niagara County
Duties & Responsibilities
- Reviews patient Nurse’s Aide Care Plan and reports change in patient’s condition to RN/LPN and/or Supervisor
- Assist patient’s with showering and personal care assistance (including oral, hair and nail care)
- Assists with transfers to toilet and records output if necessary
- Empties drainage collection devices (foley catheters, leg bags, etc.)
- Application/removal/disposal of incontinence briefs and related products
- Performs peri-care in a manner consistent with standards of infection control and hygiene.
- Prepares patient for meals, assists in setting up and feeding patients, serves fresh water and nourishments when indicated.
- Lift, transfer, and transport patients in wheelchair, bed or gurney as directed using proper technique
- Assists with patient positioning and repositioning (turn and positioning program)
- Obtain and record patient’s temperature, pulse, respiration, blood pressure, urine testing, weight, intake/output as directed by the charge nurse and/or care plan.
- Maintains confidentiality of patient information and data when reporting to the nurse.
- Responsible for routine bed making and linen changes
- Application of support devices, adaptive equipment, & oxygen.
- Post mortem care as directed
- Demonstrates sensitivity to patients to ensure their comfort, privacy and well-being.
- Participates in laundering personal patient clothing as directed
- Other duties as assigned by the nurse.
Qualifications
- High school education or equivalent
- A sense of personal responsibility and integrity
- Emotional maturity & positive attitude
- Good communication, phone etiquette and interpersonal skills
- Respect for the quality of life
Licensure/Certification
- Current NYS HHA certification required
