Healthy Start Program Provides education and support during a woman’s pregnancy, developmental screenings for children ages 0-3 years old, and makes referrals to help meet the needs of the women and children. The goal of treatment is to have a healthy pregnancy and healthy child development. Services are provided in home, through telehealth, or telephonically.

Overview:

The Healthy Start Care Coordinator provides care coordination, case management, education, psycho-social counseling and referral/follow-up to women during pregnancy and to their infants after delivery and up to the age of three years. The coordinator position also provides therapeutic services to clients within scope of training, protocols, and competence, and within requirements of statutes applicable to this position. Therapeutic services include but are not limited to assessment, service planning, direct service provision, person centered services and management of client risk and safety. The Healthy Start Care Coordinator exercises independent judgment and works with minimum supervision. Work may be performed in a variety of settings including office and client’s home.

Role Requirements:

• Bachelor’s Degree in one of the following: Social Sciences, Nursing, Health Education, Health Planning, Health Care Administration, or Social Work;
• OR Two (2) years of college with three (3) years of public health/maternal-child health experience.
• Proficiency in computer applications; Detailed orientated, quick learner, self-motivated, and deadline oriented; Experience working with diverse communities, organizations and agencies.
• Bi-lingual preferred.
• Must be able to pass organization screening requirements, including state or federal background screenings as appropriate.
• Must possess a valid driver’s license from the state in which you reside, a good driving record and be insurable under the corporate policy.

Compensation:  

The salary for this position is $19.75 per hour.
