Job Description:Johnson County Developmental Supports (JCDS) is currently seeking qualified candidates for the position of Health Supports Assistant for their nursing division. JCDS serves people with intellectual and developmental disabilities. We focus on individuals’ abilities, provide choice driven supports, and advocate alongside people to live and work in our community. Health Supports Assistants (HSA) play a vital role in supporting the coordination of our individual’s medical appointments and follow-up care. HSA’s act as a liaison between the individual’s physicians and circle of support. Many of the daily tasks in the medical department are dependent on the efficient communication and performance of the HSA's. First review of candidates will be Tuesday, December 24, 2024.
- Help schedule and transport individuals to and from all medical appointments including doctor visits, PT/OT/Speech, labs etc.
- Accompany individuals for urgent and/or emergent treatment (this position will include on-call hours)
- Provides or assists with personal care needs for individuals while on appointments; may need to provide crisis interventions, including physical intervention and restraint to ensure the safety of individuals; and administers medication as prescribed
- Advocate for proper medical treatment
- Document and communicate the outcome of medical visits with family, staff and pharmacy.
- Report and document new diagnosis and/or medical concerns
- Maintain medical records and upload corresponding paperwork into EHR.
- Ensure health outcomes are met and follow up care is completed
- Complete the process of patient admission and discharge from any outside facilities or programs, hospitals, rehab, skilled nursing, hospice or palliative care.
- Develop and maintain a positive and effective relationship with service recipients, families, and other service providers, serving as the main point of contact.
- Communicate with the pharmacy providing information on new orders or helping to get in touch with doctors for refills
- Day shift/flexible hours to attend scheduled appointments and procedures as needed
Minimum Job Requirements:
High school diploma or equivalent and one (1) year of experience working in a human services or medical field are required. KS/MO driver’s license and good driving record are required.
Johnson County Government requires reference/background screening for all positions. Specified criteria may vary by Department/Agency.
Preferred Job Requirements:
Experience in the IDD field, DSP, CNA, CMA or LPN.
Education/Experience Substitutions:
Education may be substituted for experience.
Physical, Environmental, and Special Working Conditions:
Lifting: requirement varies by assignment.
Exposure to hazardous materials (bodily fluids).
All County employees may be called upon to assist other departments in a declared emergency situation
