**JOB SUMMARY:**
Serves as the Health Project Coordinator for the Monroe County Community Health Improvement Workgroup as well as for policy initiatives at the Center for Community Health & Prevention. With minimal direction from the Director and latitude for independent judgment, the Community Health Policy Coordinator provides support as liaison with multiple representatives of Monroe County hospitals, government, public health and community. Based on the Director and specific project priorities, the Coordinator updates materials and plans, and establishes and maintains relationships with community partners/agencies throughout the year. The Coordinator plans and implements follow-up communications and data-gathering, troubleshoots and problem solves communication and project implementation barriers, provides verbal feedback to partners, and provides updates to the supervisor on a regular basis. Work includes preparation of evaluation and local, state and federal reports.
**TYPICAL DUTIES:**
+ Facilitate the hospitals’ implementation of the goals and objectives established in the Community Health Improvement Plan (CHIP) and the Community Health Needs Assessment (CHNA). Provide leadership and knowledge to inform the policy changes and implementation strategies suggested in the CHIP both within organizations and at the local and state level as appropriate. Represent the CHIW to other community groups and initiatives that involve policy especially as related to the CHIP. This may include involvement with Systems Integration, HealthiKids, UR Government and Community Relations other coalitions and projects, and city or county policy initiatives.
+ Plan and coordinate meetings and activities of the Community Health Improvement Workgroup (CHIW) which includes representation from the hospitals, health department and prominent community organizations under the supervisor’s direction. Responsibilities include setting agendas, inviting speakers, assuring attendance and gathering materials. Plan and coordinate the implementation of the projects and advocacy efforts identified in the CHIP. This may involve researching best-practices and evidence-based interventions, attending community meetings and forums, meeting with hospital champions to assure implementation, apply for grants and programs relevant to the improvement plan, meeting with policy leaders, and evaluation of the outcomes for quality improvement.
+ Under the general guidance of the supervisor, and with collaboration from the county's hospitals, health department and community organizations, the person in this role will monitor the community health needs in Monroe County and coordinate a plan for intervention every three years, report on that CHNA and CHIP to the hospital leadership, state and federal offices as requested on behalf of the Monroe County Hospitals and coordinate the progress on implementation, track and evaluate the projects, note barriers and successes to the team. Coordinate the transmission of this information to the hospitals in accordance with their completion of the 990s annually.
+ Lead policy work for the CCHP by supporting service area leaders in their policy needs. Be pro-active in linking CCHP to external advocacy efforts and in advocating on behalf of the CCHP policy needs. This will include maintaining the website health policy information, informing employees about relevant health policy topics, contributing to grants and projects that involve health policy (or health in all policies).
+ Assist the Director with the efforts needed in coordinating community benefits within UR Medicine, including maintaining a database of community benefits activities and helping with the reporting of those activities on the annual taxes as requested. Prepare and analyze reports, coordinate the budgeting and invoicing for current funding. Assist with seeking and applying for grants for additional funding streams. Explore, evaluate, and research important trends in community benefits. Under the direction of the PI, occasionally supervises students (undergraduate, graduate and medical) who are conducting specific evaluation or intervention projects.

**REQUIREMENTS:**
+ Bachelor’s degree with major coursework in appropriate health, social science, public health, or related field required.
+ Master’s in Public Health preferred.
+ 2-3 years of related health policy experience with significant policy or community health management experience; or an equivalent combination of education and experience required.
+ Experience with community-based projects or research.
+ Significant experience with project and team management.
+ Solid computer skills with knowledge of word processing, database utilities, internet navigation, email, and database software.
+ Ability to travel to various community-based agencies in and around Rochester, NY reuqired.
+ Excellent interpersonal, presentation, strong communication skills, including writing skills, problem-solving (analyzing communication flow and level of progress/cooperation), and a high degree of professionalism (quick responses to customer needs, rapid follow-through, punctuality, etc.) required.
+ Certification in appropriate Human Subjects Protection credentialing (e.g., HIPAA, EPRP credentials) will be required within 30 days of hire required.
