The Home Options Path to Empowerment (H.O.P.E) Program is one of our innovative community-based programs and helps individuals transition from a long-term care facility and move into their own home in the community. This innovative program uses interdisciplinary teams of social workers, community care partners, nurses, occupational therapists and social services practitioners to provide comprehensive support, life skill building, physical and emotional services and supports, and on-going case management to ensure a safe, successful and person-centered transition to community living for members.
You will work with individuals at various stages of the transition process. Helping them with supportive services, developing person centered care plans and coordinating activities to support their move into the community. What will you be doing:
- Create a comprehensive, person-centered service plan with each resident, identifying their goals for transition and community integration.
- Work with residents to develop life skills; identify services and supports; advocate and assist in obtaining all necessary equipment, medical care and supports necessary for a safe transition.· Facilitate discharge planning and transition to community with client, nursing facility staff, insurance health plan, HOPE nurse and OT and family/informal supports.
- Provide on-going comprehensive care management after the client transitions to the community, including coordinating benefits, in-home services, and community resources; schedule medical appointments; and advocate for clients as needed.
- Provide motivational interviewing to create harm reduction plans, crisis plans and 24-hour back up plans, as necessary.
- Collaborate with partner agencies to facilitate all aspects of the transition.
- Follow all program standards and procedures.
What knowledge and experience are we looking for:
- Nurses with RN licensure.
- Social Workers with Master's Degree (MA, MSW) in social work, counseling, psychology, or related field.
- Licensed Social Worker ((LSW), Licensed Clinical Social Worker (LCSW) or Licensed Clinical Professional Counselor (LCPC), preferred
- Minimum of 1 year experience in providing social services
- Interest in working directly with individuals and assisting them with transitions
- Experience in working directly with clients with severe mental health conditions, substance abuse and/or complicated physical illness preferred
- Excellent written and oral communication skills needed, with demonstrated ability to engage clients
- Computer skills required: Microsoft Office Suite; ability to learn proprietary program databases
- Knowledge of community supportive services
- Sensitivity to cultural diversity
- Ability to work with people who have multi-faceted needs
- This position regularly works in the field and must travel throughout the greater Chicagoland area (city, suburbs and collar suburbs). Daily access to reliable transportation is a requirement.
- Experience with Community Care Program (CCP) or other care management program, preferred
- Experience with Colbert and/or Williams consent decrees, preferred

Benefits:
The starting salary range for this role is $50,000-$62,000 annually and $70,000 to $80,000 for nurses; and occupational therapists. Salaries are based upon an individual's experience, education and licensing and our internal equity.
In addition to your annual salary AgeOptions makes a 5% contribution to your 403(b) whether your contribute or not!!
This is a full time role and includes ability to participate in our medical, dental and vision plans
We work in a hybrid environment and this role will have responsibilities in the office, regular weekly field work (greater Chicagoland area including collar counties) as well as the ability to work remotely regularly.
