Essential Responsibilities:

• Conduct comprehensive health assessments autonomously.
• Collaborate with primary care physicians to develop care plans focused on prevention and timely intervention.
• Communicate effectively with care teams and family members during transitions of care.
• Participate in case rounds for skilled and hospitalized members.

Required Proficiencies:

• Experience in skilled nursing or long-term care settings.
• Case management experience preferred.
• Knowledge of Medicare/Medicaid practices.

Licensure and Certification:

• Licensed as an Acute, Adult/Gerontological, or Family Nurse Practitioner.
• Basic Cardiac Life Support certification required.
