Position Summary: The Mobile Crisis Case Manager will work collaboratively as part of the CCBHC team to support recovery and overall wellness of referred clients. The case manager will assist with a client's recovery efforts through individual goal setting, advocacy, mentorship, education, and addressing/ameliorating social determinant of health barriers. The case manager is involved in care coordination amongst a consumer's treatment team as well as outreach to providers to educate on CCBHC services. The Case Manager will be responsible for responding to calls within one hour in the community and provide wrap around services and care coordination for 30 days following initial evaluation. The case manager will work collaboratively with the clinical team to ensure appropriate services are in place to meet the client/families’ needs and utilizing natural supports for safety
Qualifications:
- B.A. or B.S. degree in social work, psychology, or related field and have a minimum of two (2) years of experience in a human service profession;
- Certified in First Aid/CPR and as a Community Responder
- A minimum of four (4) years employment in the human services field may be substituted for a bachelor's degree.
- An understanding and belief in the recovery process required
- Skilled in operating various medical record software and hardware, word-processing, and database software programs
- Spanish Speaking preferred and is compensated
- Flexible schedule with some evenings
- Excellent multitasking and communication skills a must
- Ability to work independently and part of a team
- Bilingual skills are compensated by an additional 6%, above base pay.
Physical Requirements: This position requires community visits, employees in this position must have the ability to:
- Current driver’s license, reliable transportation, registration and auto insurance
- Travel to and from clients’ residence, community locations and office site, which could include using walkways, stairs and/or elevators
- Ability to lift up to 20lbs
- Must be able to work remotely and in person adhering to PHI requirements
