A leading provider of risk adjustment, quality improvement, and value-based solutions is seeking a compassionate Nurse Practitioner to join their provider network. In this role, you will conduct in-home wellness risk adjustment assessments for Medicare and other populations, focusing on the health and well-being of beneficiaries to ensure accurate risk adjustment coding.

Responsibilities:

• Perform annual in-home wellness visits and comprehensive health assessments.
• Deliver patient health education and build trust through exceptional communication.
• Assist in closing quality care gaps and collaborate to enhance care delivery.
• Travel within a 55-mile radius for member visits.

Qualifications:

• Valid NP License for the state of practice.
• Previous in-home risk assessment experience preferred.
• 3 years of patient care experience preferred.
• Access to reliable transportation.
• Bilingual is a plus.
