Position Overview
The Field Assistant – Wound Care Support provides logistical, operational, and administrative support during patient visits. This role ensures efficient, compassionate, and legally compliant wound care delivery on behalf of Omni Wound Physicians.
This position involves travel to patient homes, clinics, and healthcare facilities, assisting providers in care delivery while ensuring proper documentation, scheduling, and coordination. The Field Assistant operates within the scope of certification or training and complies with state and federal regulations. Certain clients may have intellectual disabilities and may exhibit behaviors requiring patience, understanding, and specialized communication techniques.
For per-diem roles, team members must be available to work at least four shifts per month to maintain employment.
Key Responsibilities
Clinical and Operational Support
Assists physicians with wound care procedures, including:
Preparing and sterilizing medical supplies and instruments.
Maintaining aseptic techniques during wound dressing changes.
Taking photographs of wounds to document treatment progress.
Providing hands-on assistance with basic clinical tasks, such as applying dressings and ensuring patient comfort during procedures.
Supports minor non-invasive wound care procedures as directed by supervising providers, adhering to the scope of practice.
Administers approved medications as directed by the physician, within the scope of practice.
Adheres to physician instructions regarding patient care, seeking clarification from the Director of Field Operations or Chief Medical Officer (CMO) when needed.
Works effectively with clients who may have intellectual disabilities, applying de-escalation techniques or other communication strategies as required.
Patient Interaction and Education
Communicates with patients and their families about wound care instructions, aftercare, and follow-up visits in coordination with the physician.
Provides patients and families with clear education on wound care management and preventive measures.
Demonstrates cultural competency by respecting diverse backgrounds and tailoring interactions to meet individual needs.
Collaborates with the interdisciplinary team to ensure patients receive comprehensive and coordinated care.
Emergency Preparedness
Assists in medical emergencies by following established protocols and providing support within the scope of practice.
Responds to unexpected patient needs or logistical challenges with a solutions-oriented mindset.
Documentation and Reporting
Documents procedures, patient interactions, and physician instructions accurately and promptly in the designated electronic medical record (EMR) system.
Participates in quality assurance activities, including data collection and reporting, contributing to the continuous improvement of wound care services.
Maintains detailed, compliant records of wound care treatments and patient interactions.
Mandated Reporter Responsibilities
Acts as a mandated reporter for suspected elder, dependent adult, or child abuse, as required under California law.
Notifies Omni leadership and collaborates with internal partners to ensure accurate and thorough investigations and reporting.
Submits mandated reports to government agencies when required or permitted, while prioritizing collaboration with Omni leadership.
Attendance and Availability
Arrives on time for scheduled shifts, including pre-determined start times for patient visits and travel.
Travels up to 36 miles to the meeting location to round with the assigned provider, using personal or company-provided transportation.
Communicates proactively about delays, absences, or scheduling changes.
Maintains flexibility to accommodate occasional weekend shifts or last-minute changes.
Follows the company’s call-off procedures, notifying the manager at least two hours before a missed shift.

Scheduling and Logistics
Organizes and confirms patient appointments, ensuring efficient travel routes and adherence to schedules.
Accompanies providers in a company-provided vehicle, assisting with navigation and ensuring timely patient visits.
Team Collaboration and Ethical Practices
Collaborates effectively with interdisciplinary team members, including physicians, nurses, and social workers, to coordinate comprehensive patient care.
Actively contributes to workflow improvements and process optimization.
Upholds the highest ethical standards in all professional activities, maintaining patient confidentiality and integrity.
Professional Development
Engages in continuous professional development by attending workshops, seminars, and training sessions related to wound care and patient support.
Maintains current certifications and stays updated with the latest best practices in wound care.
Other Responsibilities
Performs additional tasks as assigned to support team operations and improve patient outcomes.
Qualifications and Expectations
Required Certifications
Certified Nursing Assistant (CNA): Provides basic patient care and supports wound care procedures.
Medical Assistant (MA): Assists with patient care, administrative support, and preparation of instruments.
Licensed Vocational Nurse (LVN): Administers medications, assists with wound care, and provides patient education.
Surgical Technician: Ensures sterile techniques, prepares instruments, and handles post-procedure cleaning.
Emergency Medical Technician (EMT): Manages emergency situations and supports wound care procedures.
Preferred Certifications
CPR Certification: Preferred to ensure safety during patient care and emergencies.
Crisis Prevention Intervention (CPI) Training: Preferred for working with clients exhibiting challenging behaviors or intellectual disabilities.
Skills and Competencies
Strong organizational and time management skills.
Proficiency in using computers, handheld devices, and electronic tools for documentation, scheduling, and reporting.
Typing proficiency of at least 50 words per minute.
Ability to effectively communicate and write in English.
Bilingual proficiency in additional languages (e.g., Spanish, Mandarin) is preferred but not required unless otherwise specified based on regional needs.
Compassionate and professional demeanor with a focus on patient care.
Work Requirements

Authorization to work in the United States and provide proof of certifications or credentials as requested.
