Position Summary:
The Fetal Medicine Program Coordinator serves in a navigation role and directs care to the patients referred to the Fetal Treatment Center (FTC) Oakland. The Coordinator has an in-depth knowledge and understanding of pregnancy complications and/or congenital anomalies, the major treatment modalities, symptom management, and patient education. Under general supervision of the Manager of the FTC, the coordinator works independently to organize the clinical care for women referred to the FTC. The Coordinator utilizes a high level of clinical knowledge to screen patient referrals, triage telephone calls and emails from patients/families; review outside medical records, coordinate patient evaluations, perform duties consistent with follow up care for patients as requested by the FTC providers, ensure efficient patient care flow, act as the liaison between referring service, FTC SF, outpatient administrators and clinical staff to ensure continuity of care for patients and their families, and assist in the direction of care.
The Fetal Coordinator assists in the implementation of patient care standards and practices, works collaboratively with FTC Physicians to coordinate patient care based on standards and Care Pathways for specific fetal diagnoses and works on assigned quality projects and collects data for the fetal database. The Coordinator works with management to assist in optimizing the use of electronic medical records and maintaining thorough documentation in the system.
The Fetal Coordinator utilizes an interdisciplinary approach across the continuum of care with adherence to National Patient Safety Goals set forth by regulatory requirements. The Fetal Coordinator works as an integral part of an interdisciplinary team of Perinatologists, Neonatology, Cardiologists, Surgeons, Radiologists, Clinic Staff, Fellow, and all other ancillary departments, as appropriate, to ensure a high quality, coordinated, and satisfying experience for the patient.
Position Requirements:
Minimum Education: Master’s Degree required (Nursing, Case Management, Public Health or related field)
Minimum Experience: Three (3) or more years of recent experience in fetal medicine, neonatal/pediatric intensive care or high risk perinatal required. One (1) year of case management experience, preferred
License Required: Licensed as a Registered Nurse in the state of California (if RN).
Certification Required: Collaborative Institutional Training Initiative (CITI) training (or within 6 months of hire)
Knowledge, Skills, and Abilities: Ability to work with a diverse group of Pediatric subspecialists, Perinatologists, and Obstetricians required. Excellent organizational/project management skills required. Fluency in Spanish, preferred. Billing/authorization experience, preferred. Data system design and management experience, preferred. Experience with quality improvement systems, such as Lean, preferred. Strong interpersonal skills. Demonstrated ability to work as part of a multidisciplinary team. Demonstrated ability to work with families
Technical Knowledge: Competence with computer systems including Word, Excel, PowerPoint. Experience with EMR use required (EPIC preferred)
