A well known Health Services located in Philadelphia, PA has an immediate need for a Family Based Therapist.
Type: HC Full-Time/ PERM
Location: Philadelphia, PA
Summary:
• To promote the mission to help individuals achieve a more independent and fulfilling life by being an effective team member.
• Provide intensive family based services as a team member with a mental health worker in an effort to prevent out of home placement and improve family relations.
• Services are delivered primarily in the family's home and include: individual, family and group therapy; assessments; service coordination; and crisis prevention/intervention
GENERAL RESPONSIBLITIES:

• Maintain an active dialogue with your supervisor
• Actively participate in individual supervision/team meetings/department meetings
• Provide timely feedback as to the status of the work you are doing
• Provide input and raise questions
• Ensure your knowledge of agency and program policies and procedures
• Remain open and understand the rationale for decisions made. Follow through on decisions.
• Complete task assignments within required time frame
• Maintain high morale and cooperate with your team and with other departments

• Ensure that quality services are continually provided:
• Identify programmatic issues, develop recommendations and present them to your supervisor
• Keep your supervisor informed of progress and issues
• Adhere to your schedule to assure appropriate coverage at all times
• Respond in a timely manner to program/departmental emergencies and unusual circumstances and inform your supervisor as soon as possible
• Be conscientious and exhibit cost consciousness
• Work cooperatively with other departments within PATH
• Represent PATH and your program at both internal and external meetings, as requested
• Submit leave requests in accordance with department procedures to assure that the needs of the department can be balanced with individual requests
• Complete timekeeping per agency procedures
• Attend all mandated trainings within required timeframe and request approval for staff development opportunities as relevant to promote growth and skill building
• Performing all other duties as assigned
SPECIFIC DUTIES:
• Provide initial and on-going comprehensive assessments of identified families and youth
• Provide individual, family, and group therapy with identified families using ecosystemic structural family therapy model.
• Work collaboratively with assigned mental health worker team member to provide comprehensive services and linkages
• Provide 24/7 beeper coverage on a rotating basis
• Deliver services in a manner convenient to families, this includes evening and weekend service provision
• Consult with psychiatrist on a regular basis and attend psychiatric appointments with the youth and family
• Collaborate on a regular basis with all systems involved with the youth and family
• Provide on going role modeling and skill building to families
• Provide case management services as needed
• Attend and participate in required ecosystemic family therapy trainings in addition to required state and local trainings
• Demonstrate commitment to learning and practicing the ecosystemic structural family therapy model, by bringing videotaped sessions to supervision, utilization of genograms, eco-maps, and structural maps
• Present cases both to peers at PATH, Inc. and at training program
• Complete all documentation in a timely, comprehensive and legible manner and in accordance with PATH policies and procedures
• Contribute to any quality improvement/outcome measurement tracking and data gathering as assigned
• Meet productivity standards for your position
Skills:
• PA State Social Worker License
• 2 years experience in the CASSP system
• Master's degree in social work, counseling, marriage and family or psychology from an accredited college or university
• Good Interpersonal Skills
• Consistently display a positive attitude

• Maintain a professional demeanor
• Demonstrate integrity
