Families and Youth Innovation Plus is hiring a full-time Family Outreach Services Success Coach (Case Manager)

The Family Outreach Services Success Coach (Case Manager) role ensures timely response and acceptance of referrals from New Mexico Statewide Central Intake (SCI). The Case manager serves families who are reported and screened out for investigation for child abuse and neglect. The emphasis of case management is on assessing and addressing the family needs to better support any future prevention of child abuse and neglect.

Key Responsibilities:

• Provide clients with supportive services
• Coordinate services between providers and clients
• Interpret agency services to clients and community
• Provide case management services and skill-building services to youth in the program
• Maintain individual case records on clients receiving services
• Provide specialized case management services to individual client needs
• Educate clients on available community resources and services
• Create programs and services to benefit clients and the community
• Attend community meetings relevant to program and clients

Qualifications and Experience

• Bachelor's degree in social work or related human service field.
• At least 1 year of internship or post-graduation experience OR two years of combined experience working with communities on health or social service related matters, case manager experience.
• Must pass a CYFD Criminal Records Check and maintain a valid NM Driver's License
• Must maintain a full certification of the following trainings; CPR/First Aid and CPI
