Position Overview:
Care Medical Practice is looking for a compassionate and skilled Nurse Practitioner. In this role, you'll collaborate closely with a multi-disciplinary team to deliver high-quality, personalized care in a clinical setting. The ideal candidate is experienced in primary care, committed to improving patient outcomes, and eager to make a meaningful impact in underserved communities.
Key Responsibilities:
- In partnership with the clinical team of Care Medical, provide medical highest quality medical care for the practice’s patients.
- In partnership with the Medical Director and Management, assist and support clinical personnel (e.g., clinical nurses, allied health professionals, EMR, telemedicine operations).
- Provide consultations and perform diagnostic and therapeutic procedures in their clinical discipline within their practice scope, consulting when appropriate the supervising physicians for assistance with both diagnosis and treatment.
- Works collaboratively with a multi-disciplinary team to create individualized treatment plans for patients; embrace and promote a patient-centric model of care.
- Commit to the practice of value-based care which emphasizes care in the appropriate setting to improve and lower costs, address and close gaps in care and drive high patient satisfaction.
- Demonstrate proficiency and support use of the practice management system and electronic medical record (EMR).
- Perform other job-related duties as assigned.
Key Qualifications:
- Current New York State licensure as a Nurse Practitioner.
- Nurse Practitioner certification from ANCC or AANP.
- 2+ years of clinical experience in primary care
- Current DEA licensure
- Experience in family medicine and internal medicine.
- Strong communication and relationship-building skills.

The working environment and physical requirements of the job include:
This position requires in-home, assisted living, and independent-living community based work. The job requires frequent travel for patient visits in all types of weather conditions. Work may be performed in settings with conditioned air, artificial light, and an open workspace.
In this position you will need an ability to travel frequently by car and/or public transportation, the ability to communicate with customers, vendors, management, and other co-workers in person and over devices, sometimes with people who are agitated. Regular use of the telephone and e-mail for communication is essential. Sitting or standing for extended periods is common. Must be able to receive ordinary information and to prepare or inspect documents. Lifting of up to 50 lbs. occasionally may be required. Good manual dexterity for the use of common office equipment such as computer terminals, calculator, copiers, and FAX machines. Good reasoning ability is important. Able to understand and utilize management reports, memos, and other documents to conduct business.
