FALMOUTH/HYANNIS - Home Health Care RN - Able Home Care

Able Home Care is looking for Part-time or Per Diem Home Health Care Nurses in the FALMOUTH/HYANNIS area. There is a potential for this position to become Full-time.

Responsibilities:

• Delivers professional nursing services in the client's home setting.
• Key mission is to educate clients on disease process which includes managing medications and creating Oasis documentation.
• Prepares clinical and progress notes, submitting all required paperwork daily.
• Continuing professional development offered in learning Oasis.
• Coordinates services, informing the physician, management, and other personnel of changes in the client's condition and needs.
• Ensures quality and accuracy of patient records and charts in compliance with Medicare and Mass Health standards.
