Lancaster Nursing & Rehabilitaiton is currently recruiting a Registered Nurse for the position of RN QAPI Risk Coordinator.
Summary:
The QAPI Risk Coordinator reports to and receives general direction from the Administrator and is responsible for evaluating risks as a means to reduce injury to patients, staff members, and visitors within the center. The position works proactively and reactively to prevent adverse incidents and to minimize damages (reduce the likelihood of or reduce negative outcomes) following an event by development of QAPI initiatives and ensuring mitigation plans are in place and followed.
Qulifications:
- Graduate of an accredited school of Nursing
- Must possess a current, unencumbered Registered Nurse License issued by the State.
- Minimum of 5 year experience as a Regional Director of Nursing in long-term care setting and must be knowledgeable of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to long-term care. Or minimum of 10 years Director or nursing in long term care setting and must be knowledgeable of nursing and medical practices and procedures.
- Recent Experience in a Healthcare Risk Management role
- QAPI certification

- Skill in preparing and maintaining records, writing reports, and responding to correspondence.
- Ability to maintain quality assurance standards.
- Knowledge of local, regional, federal and state specific regulations for skilled and assisted living centers.
Job Duties & Responsibilities
- Review facilities electronic health record daily to identify high risk events.
- Validate staff understanding within the center of Risk Events/Escalation and ensure events are being evaluated and follow up appropriately.
- Actively participate with the IDT daily to review and conduct daily evaulation and audit clinical systems for complete and through documentation and QAPI review and analysis.
- Monitor newly admitted residents to identify risk areas and ensure that preventative measures are in place (elopement, falls, illegal/illicit substances, etc.).
- Conduct rounds daily to identify risk related concerns via observation, interviews with residents and staff. Issues will be addressed with appropriate department
- Provide real-time training to staff in relation to care that promotes preventative measures related to skin breakdown, falls and other accidents, resident to resident behaviors, elopement, choking, etc.
- Actively participate in and/or lead staff education sessions about resident specific care needs as it relates to preventative measures, safety, etc.
- Participate in morning clinical meeting and weekly risk meeting to assist in identifying and mitigating resident specific risk related concerns.
- Assist in investigations of events to identify route cause, determine whether patterns or concern for others exist, work with team members to collaboratively identify best practices.
