- Work collaboratively with physicians, nurse practitioners, nurses, therapists, social workers, and caregivers to deliver comprehensive and coordinated care to our patients in their homes – conduct patient home visits
- Supervise team of individuals who provide patient care
- Assess, plan, direct, and evaluate each patient’s care plan to ensure it meets the patient’s needs
- Manage emergency situations in a prompt, precise, and professional manner
- Complete medication administration and treatment needs for all assigned patients
- Provide nursing care in accordance with applicable regulations and Company policy
- Prepare and maintain accurate and complete clinical and progress notes on a timely basis
- Interprets operational indicators to detect census changes and increases or decreases in volume, which could impact staffing levels, revenues, or expenses census changes and increases or decreases in volume, which could impact staffing levels, revenues, or expenses.
- Hires, evaluates and terminates organization personnel.
- Oversees the maintenance of patient clinical records, statistics, reports, and records for purposes of evaluation and reporting of organization activities.
- Responsible for the maintenance of adequate and appropriate inventory supplies and equipment for the provision of patient services.
- Develops, implements, and evaluates the orientation program for new organization personnel. Responsible for orientation of new organization personnel, either directly or by delegating to another staff member.
- Ensures compliance with in-service and continuing education programs to meet education & training needs of organization personnel.
- Assists in the development of organization goals. Develops, recommends, and administers organization policies and procedures.
- Active RN state license
- Valid drivers’ license
- Five or more years of applicable nursing experience
- One or more years of supervisory/leadership experience
- A degree in Medicine or Bachelor's degree in Nursing from an accredited program by the National League for Nursing. Master's degree in health care preferred.
- One year of management experience in home health or related health care organization.
- Demonstrated ability to supervise and direct professional administrative personnel.
- Ability to market and deal professionally with customers and the community.
- Has excellent observation, verbal and written communication skills.
- Knowledge of business management, governmental regulations, and ACHC standards
