**Urgently Seeking**
Full Time Evening Shift 4pm-12am Hospice LVN
Must be able to cover San Fernando Valley/Ventura areas.
Hourly Rate $35 + Mileage Reimbursement

This evening shift position covers the 3pmto 11pm shift. During this shift theLVN is available to travel to visit patients at their place of residence and iscompensated for the visits made.
Under thedirection of a Registered Nurse, the hospice LVN provides skilled nursing andpalliative care to patients at their place of residence. The LVN workscooperatively with other Interdisciplinary Team members and providesintermittent visits or shifts of skilled nursing care, including painmanagement/symptom control, and nursing observations, The LVN providesinformation about the expected course of the illness, care techniques and otherhealth measures to patients, family members or primarycaregivers. The LVN participates in the care plan development andimplementation. Must be able to utilize computers and be comfortablewith electronic medical records.
QUALIFICATIONS
- Graduate of an accredited school of vocational nursing.
- Current LVN license within the State of California. Certification in Hospice and Palliative Nursing encouraged.
- At least one year of LVN experience.
- Valid driver's license, automobile insurance, reliable and well maintained transportation.
- Spanish speaking a plus, but not required
