About the Role:
As the Endocrine Nurse Care Manager, you'll play a vital role in providing specialized care management for patients with Type 1 diabetes mellitus. Working under the supervision of a licensed endocrinologist, you'll be responsible for all aspects of quality care related to diabetes, ensuring seamless communication with patients and families based on their individual needs. Your duties will include scheduling appointments, managing calls, ordering lab studies, facilitating remote monitoring, providing education on medication administration and adherence, and offering chronic disease management support.
Key Responsibilities:
Provide specialized nursing care management for patients with Type 1 diabetes, in collaboration with the primary care provider and endocrinologist.
Support achievement of patient and population targeted outcomes, advising on diabetes program development as appropriate.
Act as a resource and elevate skills and knowledge of internal teammates and departments providing care to patients with diabetes.
Manage a caseload of patients through in-person, telephonic, and virtual interactions.
Utilize clinical expertise and evidence-based practice to identify and analyze complex problems in nursing care of individuals and populations with diabetes.
Advocate for patients and collaborate with interprofessional teams to create patient-centered care plans.
Utilize technology platforms for patient interactions and data management.
Document patient care according to professional standards.
Qualifications:
Bachelor of Science in Nursing from an accredited school of nursing
Current Registered Nurse (RN) license from the Massachusetts State Board of Registration
Basic Life Support (BLS) certification
2 years of professional nursing experience, preferably in an ambulatory or primary care setting
Certification in Diabetes Management or Certified Diabetes Educator preferred
Excellent oral and written communication skills
Ability to work effectively with patients from diverse backgrounds
Strong teamwork and multitasking abilities
Reliable transportation for travel to clinical sites and work events
