Employment Specialist 099 (21757)
Description
- Works as part of a multi-disciplinary team grounded in the housing first approach, and coupled with an integrated service model to assist families in obtaining and retaining housing in Somerset, Hunterdon, Warren and Mercer County.
- Responsible for establishing a trusting relationship with families and working with them to identify their service goals.
- Holistic planning approach to assist families with connecting to, navigating and coordinating needed health and social services.
Requirements
- Bachelor’s Degree in related field required with 2 years related experience.
- Case management/documentation; rotating 24/7 coverage required.
- Resume building and applying for vocational, employment and school.
- Case conference with CP&P and other providers.
- Requires a flexible schedule including availability on evenings.
- Driver’s license needed with an acceptable driving history.
- Remote work possible after 3-month probationary period.
