- Position Summary
- Performs admission reviews on all patients at SJ/C during assigned hours. Including but not limited to calling, faxing, and on-line use with payers to ensure approved patient placement, covered days and discharge plans. Ensures timely, accurate and complete documentation of initial clinical information, which supports patient placement and healthcare services provided to patients and coding.

- Bachelor's Degree in Nursing - Preferred or Master's Degree in Social Work - Preferred
- Experience
- 1-2 Years of nursing or social work experience - Required
- 3-5 Years of case management experience in acute care setting - Preferred
- License & Certification
- Professional License with State of Practice - Required
- Licensed Clinical Social Worker (LCSW) - Preferred
- Licensed Medical Social Worker (LMSW) - Required within 2 years of hire
- National Certification in Case Management - Preferred
- Core Job Functions
- Works with ED staff and physicians to identify patients for potential admission requiring case manager intervention. Discusses alternative solutions to admission with ED physician and/or admitting hospitalist such as home care, clinic visit, DME, SNF, and hospice care. Collaborates with care team to manage patients who do not want aggressive medical treatment. Develops plans and strategies to manage the frequently seen in the emergency department or the re-admitted patient.
- Collaborates with physicians, nursing, and interdisciplinary team to provide insight on appropriate level of care prior to bed placement. Confers with Utilization Management team member and CCC leadership of relevant clinical information for proactive communication to payers and determination of admission status. Identifies patients out of network and with physician approval facilitates transfer to appropriate facility.
- Works with emergency department staff to identify and intervene on barriers impacting throughput in the emergency department. Follow payer requirements and government regulations to ensure compliant, safe, and cost-effective care. Documents avoidable days according to department standard operations.
- Performs brief assessment, readmission risk assessment, and 6 clicks mobility assessment within 1 business day of admission. Discuss discharge planning needs with patient, family, and care team. Ensure patient/family/caregiver is aware of and agreeable to expected day of discharge. Re-evaluates admitted patients boarding in the emergency department to determine readiness for discharges versus need for continued stay.
- Maintains communication with care team by checking voice mail and email at minimum every 2 hours, completes required documentation by the end of the workday, adheres to on-call schedule and works holidays/covers weekend WOW absences as scheduled, and provides handoffs by email to team members as patients transition through the continuum of care. Provides weekend / on-call / PTO handoff
- Maintains knowledge of laws regarding CPS/APS/Domestic Violence reporting as well as PASRR and 1013 changes/updates. Abides by NASW Social Work Code of Ethics. Consults with Social Work Supervisor for supervision when in need of education/guidance.
