Position Summary:
Join our dedicated Care Team at Sage Health, where you will work collaboratively with a Physician, Nurse Practitioner, Care Coordinator, Medical Assistant, and Nurse Care Manager. As a Nurse Practitioner, you will play a vital role in delivering longitudinal care and fostering meaningful relationships with our patients. This approach not only builds trust but also helps our patients feel engaged and connected to their care plans.
You will be responsible for providing and coordinating care for a select panel of patients within our community. Your contributions will include performing annual health assessments, conducting post-discharge visits, and delivering high-quality, cost-effective care to patients in our centers and their homes.
Primary Responsibilities:
- Evaluate and treat patients in accordance with established standards of care.
- Coordinate care with other healthcare providers, specialists, and care facilities.
- Conduct comprehensive in-home assessments for designated patients to assist in managing chronic diseases effectively.
- Perform physical examinations, medication reviews, vital checks, lab draws, wound care, and depression screenings.
- Communicate assessment findings to inform the Primary Care Physician (PCP) of potential care gaps.
- Collaborate with Medical Assistants to maximize direct patient care time.
- Demonstrate a strong commitment to serving the aging and elderly population with compassion.
- Educate patients on disease processes, medication management, and compliance.
- Adhere to all HIPAA regulations and maintain the security of protected health information.
Qualifications:
- Current and unrestricted Nurse Practitioner (NP) or Physician Assistant (PA) license (or eligibility).
- Experience in risk adjustments and/or home assessments is preferred.
- Proficient in HCC (Hierarchical Condition Categories) documentation, ICD-10 coding, and Health Risk Assessments (HRAs).
- Collaborative mindset with a passion for teamwork and working alongside talented colleagues.
- Graduate from an accredited Nurse Practitioner or Physician Assistant program.
- Current DEA certificate required before the start date.
- BLS certification is mandatory.
- Minimum of two years of experience as a Nurse Practitioner or Physician Assistant.
- At least two years of experience in Primary Care or Adult/Geriatric medicine.
