About the Role
As the ECM Clinical Manager, you'll play a pivotal role in overseeing and guiding our Lead Care Managers to address the diverse medical, behavioral health, and social needs of our clients. Your responsibilities will include direct communication with Lead Care Managers, occasional patient interactions, crisis management, and collaboration with the broader care team. You'll monitor team progress, provide guidance, and ensure the delivery of high-quality care management services.
What You'll Do
Supervise a team of Lead Care Managers, ensuring exceptional care management services.
Collaborate with the care team to create individualized care plans and monitor progress.
Support the achievement of key performance indicators (KPIs) and contribute to process improvements.
Flexibly engage in direct care management as needed to support organizational goals.
Your Experience and Qualifications
Availability: Monday to Friday, 8:30 am - 5:00 pm Pacific Time, with flexibility for occasional afternoons, evenings, and Saturday mornings.
Professional Licensure: RN, LCSW, or equivalent, licensed in California, with the ability to obtain additional state licensure as required.
Leadership Experience: Proven track record in leadership roles within rapidly growing organizations.
Care Management: Two to three years of experience in care management.
Organizational Skills: Strong ability to organize and prioritize tasks.
Independence: Capable of working autonomously while also contributing to a team environment.
Technical Skills: Proficient with Microsoft and Google applications.
Communication Skills: Excellent verbal and written communication skills.
Motivational Skills: Strong ability to motivate and resolve conflicts within a team.
Connectivity: Access to a reliable internet connection.
