The Home Care Coordinator Discharge Planner specializes in coordinating home care services and resources for patients transitioning from hospital to home, acting as a discharge planning resource without providing hands-on clinical care. BS preferredLicensure in clinical discipline (RN, PT, OT) in the State of MarylandActive CPR (Registered Nurse, Physical Therapist, Occupational Therapist) WORK EXPERIENCETwo (2) years home care or home care discharge planning experience preferredKnowledge of Medicare Conditions of ParticipationThorough knowledge of home care community resources, agency policy and procedures, federal and state regulations and third party regulations for reimbursement of home care services. ***Authorization to Work in the United States: Applicants who require sponsorship now or in the future will not be considered for this position. Salary Range: Minimum $34.00 per hour - Maximum $50.92 per hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility. In cases where the range is displayed as a $0 amount, salary discussions will occur during candidate screening calls, before any subsequent compensation discussion is held between the candidate and any hiring authority.
