Role Overview:

As a Discharge Planner, you will coordinate discharge planning services, conduct psychoeducational sessions, and establish community agency relationships to support individuals transitioning back into the community.

Qualifications:

• Bachelor’s degree in social work or related field preferred; high school diploma with experience considered.
• Experience in developing transition plans for individuals with medical or mental health issues preferred.
• Must maintain CPR certification and be an LVN.
