Position: Director of Social Services
Location: Miami, FL

Key Responsibilities:

• Conduct in-depth interviews with residents and families to gather detailed social histories and ensure personalized care.
• Collaborate with residents and families to create and implement tailored social service programs.
• Identify and resolve social and environmental challenges, offering solutions such as financial assistance, discharge planning, and coordination with community organizations for specialized services.
• Work closely with the administration, medical and nursing teams, and other departments to design and execute comprehensive social service programs and engaging activities.
• Actively participate in facility surveys and inspections conducted by authorized government agencies, ensuring compliance and quality standards.
• Serve as a resident advocate by communicating their social, psychological, and emotional needs to the care team, including physicians and other medical professionals.

Requirements:

• Minimum 3 years of experience in social services, ideally within a skilled nursing facility (SNF).
• Exceptional interpersonal, communication, and problem-solving skills.
