York Nursing and Rehabilitation Center is a 240-bed long-term care facility (nursing home). We are committed to being the leading provider of quality long-term care and related healthcare services. We are seeking an experienced Director of Social Services to join our committed staff and provide exceptional care to our residents.

Responsibilities

• Manages the Social Service Department with expertise in matters of resident advocacy, resident's rights and state/federal laws and regulations.
• Demonstrates critical thinking in the identification of social and safety issues that may occur during the course of residents' stay. Demonstrates initiative and seeks formal and informal opportunities to improve practice.
• Develop and implement written, interdisciplinary care plans for residents from admission to discharge.
• Organize and facilitate meetings for residents' families to improve residents' overall well-being.

Required Qualifications

• Social Work Degree (LBSW, LMSW, LCSW), Bachelor's (required) or Master's (preferred)
• Prior experience in long-term care REQUIRED
• Experience with PASSR, 408, and discharge planning REQUIRED
• Possess effective team problem-solving and interpersonal communication skills
