We are hiring a Director of Social Services- MUST BE A LICENSED SOCIAL WORKER in Massachusetts and have Skilled Nursing / Nursing Home experience, healthcare experience minimally, for consideration.
Responsibilities Include:
- Directs and monitors the social work services in the center.
- Promotes the psychosocial wellness of the residents.
- Actively participating in resident assessments and discharge planning to allow the resident to optimize healthcare benefits.
- Promotes a safe discharge back into the community.
- Maintains and communicates the center's discharge calendar.
- Responsible for the MOLST function with admissions.
- Coordinates and arranges for resident's care planning meetings ensuring that the resident and responsible persons are involved as able.
- Initiates discharge plans in cooperation with the resident's goals and barriers to community re-integration.
- Develops plans in cooperation with the resident accordingly to address barriers.
- Works closely with community resources such as Home Health, Hospice, local housing authorities, shelters, and day treatment providers to ensure resources are available as needed per discharge plan.
- Discusses with resident and family members/responsible party regarding Advanced Directives.
- Thoroughly and accurately complete assessments
- Function as the ACO liaison and coordinates case management tasks.
- Facilitates the 72-hour interdisciplinary meetings.
Qualifications Include:
- Bachelor’s Degree in Social Work, Master’s Degree preferred.
- Three years’ experience in geriatric, hospital, administrative or community social work.
- Must have current Social Work licensure in MA.
