Qualifications:

Required Skills:

• Clinical license required (PT, OT, SLP, or RN) or Licensed Clinical Social Worker in the state of employment required. Other licensure/certification to be considered based on appropriate qualifications.
• Two years recent experience in acute care setting preferred.
• Leadership/supervisory experience required.

• Demonstrable knowledge in all aspects of clinical care of the rehab patient
• Demonstrates general computer skills including: data entry, word processing, email, and record management.
• Demonstrates critical thinking skills.

Responsibilities:

The Director of Patient Outcomes (Rehab) is the primary individual responsible for implementing and coordinating the patient assessment and evaluation process for all patients treated by the staff in the Rehabilitation Department.  Uses his or her expertise and training in rehabilitation services and independent discretion and judgment to supervise and manage the completion of the Patient Assessment Instrument for each rehabilitation inpatient. Provides oversight and clinical support in completion of data contained in the patient assessment (PAI) and Quality Indicator data for each patient to ensure accuracy in the scoring of functional status and reflection of the patient’s care needs. Direct Case Management Department, as needed. Integrates the hospital’s mission and “Guiding Principles” into daily practice.

Posted Total Compensation :The wage range for this role takes into account the wide range of factors that are considered in making compensation decisions including, but not limited to, skill sets, experience, education and training, licensure and certifications, and other business and organizational needs.  It’s not typical for an individual to be hired at or near the top of the range for their role and compensation decisions are dependent on the facts and circumstances of each case.  A reasonable estimate of the current range is $80,000.00 - $108,000.00Minimum Salary:USD $80,000.00/Yr.Maximum Salary:USD $108,000.00/Yr.
