Supervises Directly: Direct Care Staff
Job Summary: Oversees and supports all clinical services with overall responsibilities ensuring clinical processes, staff, and policies adhere to all federal and state regulations. An integral part of the agency leadership team, this role will set the accepted standard of care and clinical compliance for the organization.
DUTIES AND RESPONSIBILITIES:
- Assures all clinical functions, processes, and policies meet all state and federal regulations and rules.
- Monitors to ensure orders are documented correctly and timely. Provides training and guidance to clinical staff regarding the requirements and agency expectations of orders for disciplines and/or treatments and services. This includes but is not limited to communication with home health ordering physicians for start of care orders and timely recertification orders.
- Follows up with intake/staffing coordinators to ensure that referrals result in a timely start of care or are well documented as non-admitted referrals.
- Assumes total responsibility for the agency’s QAPI program.
- Assures coordination of care between agency care staff, including contracted providers. Assures coordination of care between Agency and external providers as required and per agency policy.
- Serves as liaison between this Agency and physicians, hospitals, and other agencies in the community regarding patient care and clinical matters
- Keeps the lines of communication open between appropriate personnel of the Agency and of area hospitals to assure the flow of patient care information.
- Provides positive attitude as example of professional behavior for staff both the office staff and direct care staff.
- Provides leadership in orientation and competency verification of agency direct care staff.
- Assures contracted direct care providers meet agency policy requirements.
- Provides direct, in field, supervision of newly hired clinicians and assures supervision of all staff is completed per regulations and agency policy including documentation of same.
- Participate in the field according to staff needs, providing spot checks and followup for specific services and/or quality improvement issues.
- Provides direct response and guidance to staff for problem resolution in patient care.
- Provides information and reports to Administrator as instructed, including but not limited to corrective actions in Plan of Correction or Quality Improvement activities. Reports will be provided timely and without requiring reminders by the administration.
- Promotes and educates regarding the concepts of infection control and universal precautions in coordinating/performing patient care activities to prevent contamination and transmission of disease and ensure adequate orientation and staff development needs are met through collaboration with the Education Department
- Work with and ensure support of the Sales and Marketing Staff to progress the company in the community, build census and grow the company in line with our Mission Statement
- Always represents the Agency in a professional and loyal manner. Always demonstrates professional deportment and communication style with the following, but not limited to, all office staff, field staff, contractors, vendors, and patients and physicians and their offices.
- Participates in, by development of and/or presentation of staff development activities and in-service meetings to educate the personnel.
- Must be prepared to support and provide all nursing clinical visit needs if necessary that ensures patient safety and clinical compliance.
- Develops and maintains training and in-services on the following, but not limited to, topics required by state licensure, clinical services, rules and regulations, agency policies and expectations.
- Initiates joint projects and presentations to improve patient care and/or education of community members on issues pertinent to agency interests.
- Participates in the development of identifying future opportunities for home care specialization.
- Serves as a member of the administrative on-call team. Responsible for reviewing on-call logs and assure follow-up completed and documented.
- Participates, as assigned by Administrator, on compilation of data for the annual agency evaluation and/or quarterly governing body review reports.
- Participates in Quality Improvement activities as assigned by Administrator.
- Trains staff and assures delegation of components of Quality Improvement as needed to meet the total expectations of this position including review of clinical and aide visit notes, orders, and documentation.
EXPERIENCE REQUIRED:
- Must have previous and proven Home Health clinical leadership experience with a Medicare Certified Agency
- Must have complete knowledge of current Industry and Medicare changes.
- Must have relevant understanding of Medicare Conditions of Participation
- Must have sound and concise experience with OASIS-E assessment.
- Should have a sound understanding of all Clinically operational KPI’s in a Home Health setting.
- Must have complete knowledge and experience with leading an agency through any state, federal, and accrediting body survey.
- Should have and understand all clinical logic and use of home health EMR systems.
- Should have budget development and monitoring abilities.
QUALIFICATIONS/EDUCATION REQUIRED:
- Registered Nurse with current state licensure
- BSN Preferred
- Experience at a Director of Nursing Level of at least three (3) year with a minimum of two (2) years in home health
ABILITIES, SKILLS, AND KNOWLEDGE:
- Demonstrated understanding of the Medicare and Medicaid reimbursement system and thirdparty payers.
- Demonstrated expertise in coordinating work activities of multi-disciplinary team.
- Excellent oral and written communications skills with proven ability to communicate with technical and
non-technical users.
- Ability to read, analyze, and interpret common scientific and technical journals, professional journals, and legal documents.
- Ability to read and interpret physicians’ orders and notes from other caregivers as well as respond to them verbally and in writing.
- Ability to effectively present information to clients and employees and community members.
- Ability to respond effectively to the most sensitive inquiries or complaints.
- Ability to define problems, collect data, establish facts, and draw valid conclusions.
- Ability to send/receive emails and use the internet for research and informational purposes.
- Ability to use software to create a spreadsheet with minimal instruction.
- Ability to use word processing software for communication and documentation purposes.
- Ability to use independent judgment to manage and impart confidential information.
- Skill to attend to multiple tasks at once which are time sensitive.
- Basic knowledge of Windows operating system and ability to advance computer skills as required.
- Ability to establish and maintain effective working relationships with fellow employees and the public.
SPECIAL REQUIREMENTS:
- Current TB skin test screening upon hire and screening annually thereafter.
- Possession of a valid state Driver's License with reliable transportation and applicable car insurance if transporting patients.
- Proof of COVID-19 Vaccination or approved medical or religious exemption.
PHYSICAL REQUIREMENTS:
- Ability to regularly sit, stand, stoop, bend, kneel, walk, balance, crouch, crawl, push, pull, reach with hands and arms, and talk and hear.
- Ability to frequently use hands to finger, handle, or feel.
- Ability to occasionally climb.
- Ability to occasionally use sense of smell.
- Ability to frequently lift and/or move up to 50 pounds and occasionally lift and/or move 100 pounds.

WORK ENVIRONMENT:
- The employee is frequently exposed to outside weather conditions.
- The work environment is highly variable, as care is provided in private homes.
- Lighting, acoustics, air quality, temperature and environmental hazards are not predictable.
- The employee is occasionally exposed to wet and/or humid conditions, household pets/animals, fumes, or airborne particles, and toxic or caustic chemicals.
- The noise level in the office work environment is usually moderate.
TRAVEL:
The employee is required to travel frequently to complete agency business.
