About the Job
The Director of Home Health is responsible for the day-to-day operations of the Home Health services. He/She will assist the Clinical Director with the overall management of the Agency. The Director is responsible for the development of the operation of the Home Health Service program. The Director of Home Health coordinates the interdisciplinary team, and all medical services performed by the branch.
Essential Qualifications:
· At least two years of experience in the administration and provision of home health and acute care services.
· Must have, as a minimum, one (1) year progressively responsible management training and experience in home health care, health services, or related field demonstrating leadership and expertise.
· Experience and knowledge regarding home health care and/or public health nursing preferred.
· One year successful experience in supervising and/or training Home Health Aide level employees.
· Ability to make independent decisions when circumstances warrant such action.
· Ability to deal tactfully and confidentially with personnel, clients, family members, visitors, government agencies/personnel and the general public.
· Leadership and supervisory ability and the willingness to work as a team with professional and non-professional personnel.
· The ability to plan, organize, develop, implement, and interpret the programs, goals, objectives, policies and procedures, etc., that are necessary for providing quality care.
· Must have patience, tact, and enthusiasm, as well as the willingness to handle difficult situations.
· Willing to seek out new methods and principles and be willing to incorporate them into existing practices.
Essential Job Functions:

· Interviews, hires, helps to orient, trains, evaluates, and supervises the home health service staff.
· Ensures that all client records are kept in accordance with regulatory guidelines; accurately documents the need for and provision of services in the manner prescribed by funders that maximizes reimbursement.
· Oversees the day-to-day operations of the branch office.
· Conducts supervisory home care visits as needed.
· Conducts or arranges for needed staff in-services and skill training.
· Consults with the Clinical Director as issues and concerns arise.
· Participates in the annual evaluations of the Agency.
· Assesses equipment and supply needs for professional staff.
· Develops, implements, and evaluates adequate staffing patterns.
· Identifies and provides for own and staff’s learning needs to increase professional competence and advance clinical knowledge.
· Assists nursing staff in meeting objectives in relation to standards of care delivered.
· Ensures confidentiality of client information.
· Investigates client concerns/complaints.
· Performs disciplinary action of Home Health staff as necessary.
· Responsible for final QA review of all OASIS assessments prior to submission
· Responsible for submitting OASIS assessments timely in accordance with agency guidelines and Medicare COP’s.
· Responsible for ensuring all orders are reviewed for accuracy and placed in order run to be processed by office coordinator within agency established timelines.
· Responsible for monitoring patient outcomes and performance improvement.
