The Case Management team is made up of 8 total people
Inpatient hospital experience, acute rehab experience preferred.
Each of the CMs carries a patient caseload of 11-13 patients, but the Director of Case Management would carry a smaller patient caseload of 3-5 patients.
Experience with interdisciplinary meetings/rounding and be able to speak to keeping length of stay and avoidable days down.
