OVERVIEW
Provides support and instruction on a 24/7 basis in an intensive care residential home in service to the medical, physical, psychological and social needs and personal goals for intellectually and developmentally disabled adults having more complex medical or behavioral needs. Work is performed under regular supervision and is guided by well-established and specific agency policies and procedures, quality assurance, professional practices and including ethical standards related to client interactions.
DUTIES AND RESPONSIBILITIES
- Provides instruction and guidance, assists and monitors residents in daily living skills and self-care such as hygiene, eating, mobility, money management, routine problem-solving , communication skills and community interactions and activities,
- Makes supportive behavioral interventions.
- Facilitates, networks and supports residents engaged in volunteer, social and leisure activities.
- Assists residents with physician appointments.
- Transports residents via agency van.
- Dispenses and records resident medications.
- Physically assists residents with mobility and basic daily living activities.
- Liaises with family members and other concerned parties on behalf of the residents.
- Greets visitors and supervises visitation to the facility.
- Enforces observation of facility/house rules.
- Monitors individual service plans in collaboration with resident; his/her family, legal guardian and/or legal representative; other service providers and the agency treatment team under supervision of the ICF Supervisor.
- Observes, records and reports residents' daily progress, behaviors and incidents.
- Prepares individual service plan documentation,
- Provides overnight awake support, assistance, monitoring and supervision to residents.
- Participates in housekeeping and meal preparation activities.
- Secures the facility per policy.
KNOWLEDGE, SKILLS, AND ABILITIES
Considerable knowledge of the practices and methods related to the instruction of intellectually disabled adults including behavior management, independent living skills training and supportive counseling; considerable knowledge of human rights of the individual; considerable knowledge of common housekeeping practices; some knowledge of the operation of electronic medical records and associated documentation; ability to physically assist residents; ability to prepare accurate resident records; ability to establish rapport with SMI individuals and their families; ability to establish and maintain an effective working relationship with other staff; ability to communicate effectively orally and in writing; ability to function calmly in stressful situations; ability to observe, assess and record individual behaviors and functioning; ability to operate a vehicle; ability to effectively respond to crisis situations both behavioral and medical.
TYPICAL EDUCATION, TRAINING AND WORK EXPERIENCE
High School Diploma, coursework in the Behavioral Sciences, and some experience with intellectually disabled persons.
LICENSURE AND CERTIFICATIONS
Valid Virginia driver's license. Certification in First Aid/CPR. Training in Therapeutic Options (THEROPS).
