The Main Direct Support Staff function is to Support Individuals in their home and Community to live as independently as possible and develop a relationship with activities that bring life-long joy, such as music, art, conversation, and relationships.
This Full-Time position is based on week & weekend block shifts which includes overnights.
Job Title: Direct Support Professional
- Direct Support Professional job responsibilities include, but are not limited to the following:
- Will provide direct care services and ensure adequate supervision in identified risk areas during an assigned shift; at an assigned home and/or in the community.
- Complete all shift assignments, tasks, house chores, cleaning and organization as assigned by the Residential Manager or Supports Directors.
- Complete all site documentation (ex. Communication Log, Medication Log, Incident Reports, household funds, resident funds, etc.).
- Conduct fire drills with complete documentation.
- Accompanies Individual and his or her professional supports on medical and dental appointments as deemed necessary.
- Communicating to the Residential Manager and-or Supports Directors of any and all incidents involving staff and/or supported Individuals.
- Assist Individuals in daily life skills.
- Provide nutritious meals and snacks and concern for Individuals diet (ie. Diabetic) and exercise.
- Ensure that food is handled safely and stored properly.
- Transport and/or assist Individual to and from appointments, activities, and/or visitations.
- Administer, monitor and properly store Individual medication as prescribed by the doctor and complete documentation.
- Provide all individuals with support in areas such as: socialization, utilizing community resources, vocational, social and interpersonal development etc.
- Follow all individuals diet (ie. Diabetic, eating/drinking/swallowing protocols) and exercise protocols.
- Follow any and all hygiene protocols as outlined in the ISP.
- Support the person/individual to live the life they want.
- Must attend and participate in all staff meetings, training, supervision sessions, as well as other related resident/program meetings (ISP reviews), as assigned by Residential Manager and Organizational Directors.
- Attend, participate, and maintain 24 - hours of yearly training, along with a completed physical (every 2 years); and CPR/First Aid training (every 12 months).
- Must attend and participate in all designated training, meetings, supervision sessions, as well as other related program meetings (ISP reviews), as assigned by Residential Managers, Supports Directors and Quality Enrichment Director.
- Recognize and respond to concerns of neglect and/or abuse.
- Maintain open lines of communication with co-workers to ensure continuity of care and consistency of site operations and individuals' program plans.
- Recognizes and responds to concerns of neglect and/or abuse utilizing the incident report form and a follow up notification to Residential Managers and Supports Directors.
- Ensure that your timesheet is completed daily and is ready to be approved by your supper supervisor biweekly.
- Perform all other duties as assigned by the Residential Manager or Supports Directors.
