Direct Support Professional Description of Responsibilities:
As a DSP working in the home and community environment you are responsible for the delivery of quality 1:1 services to ID/A individuals and implementing their goals as outlined in the Person Centered Plan. Your direct care may involve activities of daily living, community outings, teaching money management skills, facilitating social interactions, behavior modifications, and personal care of different types per individual needs. Daily documentation is required to be completed in accordance with state and program regulations. Outside of State required CDS online modules; any additional needed PPE and required training will be provided to you by Eminence Home Care.
Responsibilities
- Implementation of the individual personalized goals outlined in their Individual Support Plan.
- 1:1 services including but not limited to Personal Care, Respite Care, Home and Community Based Supports, Community Participation Support, Behavior Plan Implementation, Communication Strategizing.
- Instruct, transfer, and reinforce targeted goals and outcomes.
- Complete documentation of services electronically through EVV App.
- Objectively document results of staff interventions and daily activity.
- Maintain appropriate professional conduct at all times.
- Respond timely to any changes to treatment plan and goal revisions as directed by interdisciplinary team.
- Provides care in accordance with written instructions from professional staff.
- Reports to the Supervisor on any changes in the participant’s needs, changes or concerns.
- Participates in all required training in addition to team meetings, reporting procedures, and changes to implementation.
- Dependable and on time for all assignments.
Minimum Qualifications
- At least 18 years old
- High School Diploma or GED required
- Experience with ID/A individuals preferred
- Active Driver’s License
- Driving record below 5 points
- No presence on DDD NJ Registry
- Ability to pass NJ regulated Drug Test
- Currently have or be willing to obtain CPR/FA Certification
- Good organizational and communication skills.
- Access to a functional device to record EVV data (mobile phone)
- Proof of eligibility to work in the United States.
