Job Purpose:
To support individuals with developmental disabilities at Bergmann Center, Inc to be as independent and engaged as possible in their communities, ReSale Shop, Bergmann Center, and home. To support individuals with the development of work skills, growth of meaningful connections in their community, build relationships in compliance with their plan of service.
Duties:
*Support individual with achieving their goals in compliance with their plan of service. This includes participating in annual meetings for review, as well as maintaining daily documentation as required. Goals can be in support of daily living, meal planning, employment, etc.
*Contact supports coordinator, guardians or other team members with any concerns or updates.
Experience working with adults with mental and physical challenges.
Good verbal and written communication skills and the ability to express yourself in a clear, concise, and professional manner.
Good time management, organizational, and planning skills.
Ability to exercise good judgement and work without close supervision.
Meet the physical requirement for the position which may include; lifting, pulling, carrying packages, equipment, and consumers up to 50 lbs.
Clean criminal history and pass Community Mental Health standards for providers.

*Be available via phone after hours if needed for client emergencies.
*Communicate with supervisor any concerns or other important client updates.
*Adherence to HIPPA and all Community Mental Health trainings and expectations for provider services.
*Obtain and maintain chauffer's license.
*Be responsible for your clients’ funds that are under your supervision to include keeping balance sheet and sending balance sheet and receipts to the guardian.
Skills/Qualifications:

starting at 18 /year
