Job Description:
Under the direction of the Medical Practice Manager or designee, the DTC RN ensures quality and safe delivery of nursing services in the Diagnostic & Treatment Center (DTC) and Day Health Center (DHC).
Assesses, plans, implements and coordinates nursing care in the Medical Practice and DHC
JOB RESPONSIBILITIES:
Provides prescribed or emergency care to patients requiring professional nursing services, including: venipuncture, dressing changes, straight catheterization, suctioning, immunizations, etc.
Teaches and supervises family members, auxiliary nursing personnel, or others giving care via one-to-one instruction and educational in-services to DTC patients and staff.
Performs diagnostic tests as authorized by the Primary Care provider (PCP.)
Obtains laboratory specimens when indicated.
Provides treatment under medical direction.
Assesses, evaluates and reports to physician patient's physical and emotional condition, reaction to medications and/or treatments, and/or critical laboratory result requiring PCP intervention.
Reports pertinent patient information, and maintains up-to-date patient records.
Arranges adequate provision of supplies and medications in the Medical Practice and DTC.
Ensures proper working condition of all Medical Practice equipment (i.e., EKG, suction machine, etc.).
Tracks and monitors currency of all stock medications at the DTC. Coordinates with Nurse Manager and/or Business Manager ordering of needed stock medications.
Conducts periodic monitoring/inventory of Emergency Crash Carts and/or Emergency Box as per agency policy.
Participates in interdisciplinary team meetings and quality assurance activities.
Administers medications and treatments as prescribed by the PCP and independently performs skilled nursing procedures and techniques based the changing nursing needs that the member requires while at the PACE Center.
Teaches the patient and family/caregivers self-techniques whenever appropriate and provides instruction regarding medication, diet, safety and treatment modalities as prescribed by the PCP after the clinic visit and in accordance with the plan of care.
Knows and conforms to the law governing the practice of professional nursing and provides professional nursing care using all elements of the nursing process.
Conducts monthly Medical Practice environmental rounds to ensure area is safe for patients and staff.
Coordinates and collaborates with the Site Medical Director and members of the interdisciplinary team (IDT) the Infection Control and Surveillance activities at the PACE Center.
Supervises Nursing Attendants to maintain patient flow and conducts annual clinical assessments for Nursing Attendant staff.
Acts as preceptor for students on rotation from nursing schools.
Assists with coordination/preparation of mail order prescriptions.
Completes prescription log and prepares mailing list.
Conducts yearly Influenza immunization campaign and performs PPD tests.
Administers adult/geriatric required immunization as prescribed by PCP.
Completes, maintains and submits all required documentation that is timely (in compliance with agency policy) accurate, and relevant.
Performs other duties as assigned.
Requirements:
Current and unrestricted Registered Nurse license required
Active registration to practice in New York State required.
Graduate of an NLN accredited nursing program with a B.S.N. preferred.
Defined as at least one (1) year of experience in an acute care medical-surgical, adult/geriatric ambulatory care/clinic practice setting, and/or certified home healthcare agency (CHHA) required
Experience must be within one (1) year prior to date of hire or transfer required
Experience with EKG is also preferred
IV/Phlebotomy Certificate or strong experience is a must
Certification requirements:
The following are required certification at time of hire or transfer and must demonstrate recent verifiable experience (within one 1 year) to independently perform:
IV Therapy
Phlebotomy / Venipuncture
EKG interpretation
Basic Life Support certified by the American Heart Association or American Red Cross)
Knowledge, Skills and Abilities:
Computer/laptop proficiency required
Good organizational, observation, communication and judgment skills,
Is self-directed with ability to work with supervision.
Is flexible and cooperative in fulfilling role obligations.
