Schedule: Regular FT 40 Hours Per Week

Under general supervision of the CHC Manager, the Diabetic Educator assesses the learning needs of patients, family members and staff, providing individual and group instruction on diabetes self-management topics as needed. The educator evaluates the effectiveness of the educational process and communicates with the appropriate staff. The educator serves as a resource to patients, staff and the community in diabetes-related topics.
Hourly Pay Rate: $32.84 - $48.44
Qualifications
Education:
- Requires an Associate of Science in Nursing or Bachelor of Science in Nutrition. If RN, prefer a Bachelor of Science in Nursing.
Experience:
- Requires a minimum of three (3) years as an RN or RD.
- Prefer five to ten (5-10) years working as a Diabetic Educator.
Certification/Licensure:
- Must posses a current, valid Arizona RN or RD license; if RN, must be in good standing with the AZ Board of Nursing.
- Requires CDE certification.
- Ambulatory Clinics: Must have a valid fingerprint clearance card issued by the Arizona Department of Public Safety OR submit a completed Affidavit at time of hire. Must complete a fingerprint clearance card application within seven (7) days of the hire date. Must present actual card within 90 days of applying for fingerprint clearance card.
- Requires BLS. A BLS card obtained through an approved American Heart Association (AHA) training center is preferred and will be required within 3 months.
Knowledge, Skills & Abilities:
- Requires clinical expertise and advanced knowledge of Diabetic Education.
- Requires strong leadership, mentoring, communication, and organizational skills.
- Requires the ability to read, write and speak effectively in English.
- Requires an ability to approach patients with tact and sensitivity.
- Bilingual in English and Spanish is strongly preferred.
- Must have transportation and be willing to travel to various ambulatory health centers.
