Job Description: Direct Support Professional (Day Habilitation)
Report to: Day Habilitation Program Supervisor
The Direct Support Professional (DSP) is responsible for a variety of duties related to the overall
care of individuals with developmental disabilities receiving services from APluscare Behavioral
Health programs. This includes but not limited to caring of individuals and maintaining daily
level of support, assisting them with their goals and strategies, implementing of individual
support plans (ISP) and behavior plans (BP) in addition to promoting individual independence
and helping with total integration and inclusion into the community. Direct Support
Professionals are required to perform all duties in accordance with the Division of
Developmental Disabilities (DDD) regulations and APluscare Behavioral Health policies and
procedures.
JOB RESPONSIBILITIES
• Administer medications to individuals as prescribed
• Observes individuals for any changes in behavior or health and evidence of injury or
bruises
• Contact the pharmacist for medication refills and script follow ups
• Assist individuals with medical appointments
• Ensures individuals are dressed neatly and appropriately and are always groomed
• Prepares nutritional meals based on individual dietary needs
• Encourage individuals to assist in the preparation of meals and snacks as appropriate to
their individual dietary needs
• Maintains cleanliness of day habilitation program physical plant both interior and
exterior and individual's personal space clean, organized, and tidy and encourage
individuals to participate in keeping their space clean
• Teaches individuals daily skills such as household chores, laundry, cleaning, and washing
dishes, as appropriate according to their individual support plan
• Provides transportation to and from activities, as well as the day habilitation program
• Implementation of individual support plan and behavioral support plan goals
• Assists individuals in achieving their goal on wellness programs such as recreational,
social and community activities according their individual needs and plans
• Promotes participation and involvement in community outings
• Works all assigned schedules, including but not limited to arriving and departing on
time, requesting time off in advance, and notifying your Program Supervisor of changes
to your assigned work schedule
• Ensures all individuals rights are protected
• Treats individuals with dignity and respect
• Report any noted problems to the Program Supervisor immediately and complete an inhouse (APluscare) incident report form
• Represents APluscare Behavioral Health in a professional manner to families, service
coordinators, State officials, and community partners when required
• Documents all program data, including but not limited to individual behavior plan data,
activity logs, community integration logs and critical logs
• Documents individual's health such as seizure charts, weight, bowel movements and
fluid intake
• Ensures that all documentation is completed accurately and in a timely manner
• Ensures Medication Administration Record (MAR) entries are completed as required
• Completes communication logbooks during shift and all other duties as assigned
• Cooperation with all agency, state (DHS/DDD), and federal investigations and
inspections
• Attend all required staff meetings and trainings scheduled
• All other duties as assigned by APluscare
Shift: Monday - Friday 7:30am - 3:30pm
Qualifications:
• A minimum of a High school diploma, or GED required
• 6months-1year of experience working with people with developmental disabilities
• Push, pull, and lift up to 50lbs
• Ability to work on holidays, weekends and during inclement weather
• Complete annual trainings related to individual ISP, Behavior Plan and all other assigned
refresher trainings assigned by APluscare
• Valid Driver's License, clear driving record and driving Company's vehicle
• Must be at least 18 years of age
Trainings: All must successfully complete the mandatory DDD trainings: Overview of
Developmental Disabilities, Abuse and Neglect, Medication Administration, CPR & First Aid,
Agency New Hire Orientation, Agency's approved training on handling Life-Threatening
Emergencies/ Danielle's Law and other professionally based training deemed necessary.
