About the Job
As a Program Manager you will oversee clinical growth and awareness of Adult Cystic Fibrosis within the community and throughout the ambulatory setting and in the hospital. Collaborate with administrative and medical leadership to develop community outreach and education programs. Organize and participate in extending quality care to patients. Coordinate efforts for the growth of the program, expanding services and patient retention projects. Coordinate clinical and quality improvement studies.
Responsibilities Include:
• Collaborates with administrative and medical staff leadership in defining challenges, opportunities and initiatives for the Adult Cystic Fibrosis program.
• Works to enhance outreach and education efforts within the community. Coordinates and organizes internal and external events including developing and implementing an itinerary; scheduling of staff and volunteers; securing available resources, supplies and locations; and ensuring coordination of all involved.
• Collaborates with the clinical team to determine treatment plans for Adult Cystic Fibrosis program patients. Reviews and documents evaluations, assessments, diagnostic testing and other appropriate data.
• Participates in quality meetings to discuss cases and review clinical outcomes of Adult Cystic Fibrosis program patients.
• Organizes clinical pathways to promote quality patient care and optimize outcomes.
• Ensures follow-up communications with patients after their visits, and leads efforts to improve patient retention and recruitment.
• Meets the educational needs of families and other health care professionals concerning the care of the patients.
• Provides education and training to patients about their condition, including self management strategies.
• Organizes clinical studies and presentations necessary for patient approval, based on clinical algorithms and inclusion criteria.
• Coordinates with billing and medical staff to capture and optimize charges related to Adult Cystic Fibrosis program services.
• Collaborates with Health System/Hospital staff to complete analyses, draw conclusions, develop recommendations and synthesize information into final product to maximize profitability of Adult Cystic Fibrosis program program.
• Serves as a clinical liaison between the center and vendors for all procedural and point of care testing that is performed at Adult Cystic Fibrosis program.
• Graduate from an accredited Nurse Practitioner Program required with Master’s Degree in Nursing, preferred.
• Licensed Registered Nurse Practitioner by New York State Department of Education OR current license to practice as a Registered Nurse Practitioner in New York State, required.
• Certified or experienced in Adult Cystic Fibrosis program nursing, preferred
• Minimum of five (5) years clinically related direct patient care experience in Adult Cystic Fibrosis care, preferred.
• Prior experience in program development and research, preferred.
• Excellent communication and interpersonal skills, required.
