About the Job
The individual in this role will serve each client at Ozarks Healthcare Crisis Stabilization Center by assisting them in connecting with community resources and outpatient care facilities. The focus of OZH-CSC is to provide urgent care psychiatric services to our seven county catchment area. The goals of our center include: rapid access to psychiatric and therapeutic services, development of integrated health services, reduce burden on local ERs and law enforcement for behavioral health crisis, and increase availability of psychiatric beds. The CSC will strive to assist in stabilizing the individual by utilizing both nursing and behavioral supports offered on a time-limited basis.

Identify and coordinate appropriate referrals throughout client stay and upon discharge. These referrals may include: access to shelter and transitional housing, HUD housing, Medicaid, food stamps, employment applications, and other applicable community services | 30% | ||
Participate in client advocacy to help each client overcome their own unique stability barriers by assisting to develop natural supports within the community | 30% | ||
Document on client encounters, treatment progress, and discharge outcomes | 20% | ||
Coordination and collaboration with CSC nursing staff and CSC Qualified Mental Health Professionals to identify client’s individualized strengths, needs, abilities, and preferences in order to encourage client awareness, independence, self-care, responsibility and readiness for change | Ongoing | ||
Model and educate clients about daily living skills, educating them regarding diagnosis and treatment, accessing services and resources as needed | 20% | ||
